What to expect at your first visit

Your first appointment is all about getting to know you and understanding your oral health. It typically includes a comprehensive exam, any necessary digital X-rays, an oral cancer screening, and often a cleaning. Dr. Chaw will review his findings with you, answer your questions, and discuss any recommended treatment, clearly and without pressure.

What to bring

  • Photo ID (driver's license or similar)
  • Your dental insurance card, if you have coverage
  • A list of any medications you take
  • Relevant medical history and your previous dentist's contact info
  • Any questions or concerns you'd like to discuss

New patient paperwork

To save you time in the waiting room, we'll provide your new patient forms before your visit. When you request an appointment, our team will let you know how to complete your paperwork, either online ahead of time or when you arrive a few minutes early. Please don't include sensitive medical details in our website forms. We'll collect your health history securely through our patient paperwork.
